  • HEALTH INTAKE FORMS: We will continue to require current and new clients to completely fill out and submit the online Health Intake Form prior to scheduled sessions. These forms are sent in a separate email after appointment confirmation.
  • BOOKING FOR ANOTHER PERSON: Each client is required to have individual accounts. Please enter that person's information when making an appointment.

Upon confirmation of your appointment, please fill out and sign the online Client Health Intake (new clients) or the Health Update Forms sent in a separate email.
This form must be filled out completely and submitted before your scheduled session.

To guarantee availability, please allow at least 24 hours of advanced notice when setting up appointments.

​We require AT LEAST 24 hours of advanced cancellation notice.
No shows and last minute cancellations will be charged the full session rate or automatic redemption of vouchers/gift certificates.

*** If you are running late for your scheduled appointment, please call or text us at 503-477-2857 as soon as possible. However, your session may need to be adjusted to the time remaining (but no less than 45minutes). If we do not hear from you within 15 minutes from your session start time, your appointment will be automatically cancelled and you may be charged for the full session cost (or automatic redemption of certificates or pre-paids).

*** Illness and emergencies happen, and for these reasons we understand last minute schedule changes. However, no shows and habitual cancellations prevent others from scheduling their sessions for that time, which affect our income. Please be courteous and do not schedule unless you are committed to that time or let us know at least 48 hours in advance if you need to reschedule. Thank you.

*** Massage should not be considered as a replacement for professional medical treatment: a physician should be consulted in all matters relating to health and especially in relation to any symptoms that may require diagnosis or medical attention. In some cases, a primary physician’s approval/prescription is required before any bodywork is administered. Care should be taken during pregnancy, particularly in the use of essential oils and pressure points.
